Step 3: Complete sections C & D and sign the PA DL-503 form. (CDL Drivers Only)

Step 4: Complete all sections of the Erie Insurance Survey and sign. (CDL Drivers Only)

Step 5: Complete section 1 of the Safety Performance History Report, for each previous employer within the past 3 years. (CDL Drivers Only)

Step 6: Review, sign, and date all forms, where required.
